Transition Care | Follow Ups | Evaluations | Medication Assistance

We have established a medical provider group to offer evaluation and medication assistance for individuals with a diagnosed substance use disorder. You do not need to be a current Farnum client to take advantage of these services, and we accept most insurance.

Examples of how we can help include the following:

  • Outpatient withdrawal symptom management
  • Transition of care prescriptions for psychiatric and substance use treatment needs
  • Follow up until appropriate transition can be established
  • MAT services for patients receiving SUD treatments outside of Farnum MAT services
  • Consultation and follow up for referrals of SUD patients from primary care providers
  • Vivitrol and Sublocade injections
  • Assessment and referral to MAT services
  • SUD and medical needs not typically covered by MAT services
